Part I of the application form is to be completed by the parent or guardian of the applicant; Part II is to be completed by the applicant (students in grades 2-12 only). A recommendation form must be completed by the applicant's English and math teachers, as well as a coach or teacher who has supervised the student in an extracurricular activity.
