Application for grades 5-12

Application forms for grades 2-12 have two parts. Part I of the application form is to be completed by the parent or guardian of the applicant; Part II is to be completed by the applicant.

Recommendation Forms

For Preschool-grade 4 a general recommendation form must be filled out.

General Recommendation Form

For grades 5-12, a recommendation form must be completed by the applicant's English and math teachers, as well as a coach or teacher who has supervised the student in an extracurricular activity.
